Participants that choose to be taped recognize and acknowledge that there are certain hazards and risks of physical injury to participants in recreational activities/programs, events, and I hereby voluntarily and knowingly agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that I may sustain as a result of said participation or the use of KT Tape.


I, for myself my heirs, and assigns do herby fully release and forever discharge Lumos, Inc. a Utah Corporation dba KT Tape, including its owners, managers, officers and employees, agents, and volunteers, ( hereinafter collectively referred to as \u201cKT Tape\u201d) from any and all claims for negligence, injuries, including death or incapacity, illnesses, damages, expenses or loss that I may suffer arising out of, connected with, or in any way associated with the use of KT Tape, the application of KT Tape by agents or volunteers of KT Tape or the sporting event, and/or participating in the race, program, event or activities and I agree to indemnify and hold KT Tape, its affiliates and assigns, harmless from any and all such claims.


I acknowledge that the persons applying KT Tape (Tapers) may be volunteers with limited training or experience. I acknowledge that Tapers do not have any specialized medical training and I am not relying on Tapers to provide any medical treatment or diagnosis. I recognize that in being taped, I am not creating a Physician/Patient relationship. I am not relying on KT Tape or the Tapers in any way for medical advice or treatment. The undersigned acknowledgement that taping is not a substitute for professional medical diagnosis and treatment. I accept all responsibility of obtaining examinations and recommendations from my own physician prior to being taped and/or participating in this event.


I agree that I am solely responsible for determining if I am physically fit and/or skilled for the race, program, or activities contemplated when being taped. I recognize that the choice to participate is mine alone, and I hereby represent that I am physically able to participate and I assume all risks associated with the use of KT Tape while exercising.


I grant permission to KT Tape, its affiliates and sponsors to use any photographs, motion pictures, recordings or any other record of this event for and purpose including, but not limited to, promotion, advertising and marketing purposes. Any and all photographs, motion pictures, recordings or other record of the event are the sole property of KT Tape.


I have read and fully understand the above information, warning of risk, assumption of risk and waiver and release of all claims.
